Temporary/Limited Permit:

A temporary license may be granted to a person who has completed the education and experience requirements. This allows them to practice under the supervision of a licensed OT.  There is a $25 fee for OTs and $20 for OTAs.‍‍‍

Initial Licensing Fees:

OT Fee: $35, plus $21 for a Criminal History Records Check
OTA Fee: $30, plus $21 for a Criminal History Records Check

Timeline:

Renewal:

Licenses need to be renewed every year by March 31.  The renewal fee for an active license is $35 for OTs and $30 for OTAs.
