Temporary/Limited Permit:

An application for full licensure must be submitted first, and then submit an application for the temporary permit with a $50.00 application fee.  A person with a temporary permit may only practice occupational therapy or occupational therapy assisting under the direct supervision of an Indiana certified occupational therapist that is responsible for the patient.‍‍

Initial Licensing Fees:

OT Fee: $100
OTA Fee: $100

Renewal:

OT and OTA licenses expire December 31 of even-numbered years and require a $100 renewal fee.
